  1. Name the specific change: what is different now compared to your normal baseline?
  2. Note the timeline: when did it start, and has it been constant or does it come and go?
  3. Identify triggers: what makes it better, worse, or seems unrelated?
  4. Name the underlying worry: what outcome are you actually concerned about?
  5. Combine all four into one specific question to research or bring to a clinician.
