A single bad day rarely tells a clinician much. Two weeks of consistent tracking usually does. This journal structure captures the four data points that matter most: what happened, when, how severe, and what else was going on that day.
Walk Through It
Date and time of the symptom or change.
Severity on a simple 1-10 scale.
What you ate, how you slept, and your stress level that day.
Anything that made it better or worse.
Review after two weeks for patterns before your next appointment.
